Predictors of Cesarean Section in Term Nulliparous Women Presented in Labour at Tertiary Hospital: A Cohort Study

چکیده
Objective: To identify predictors of cesarean section in a term nulliparous women presented in labour. So that we can avoid unnecessary primary cesarean section and in the same time avoid, as much as we can, obstructed labour. Methods: This was a cohort study of 216 nulliparous patients presenting in labour at term with singleton vertex fetuses who were followed till delivery either vaginally (n = 144) or by cesarean section (n = 72). Multiple logistic regression was used to identify independent demographic and clinical predictors for cesarean delivery. Results: Increased maternal weight, advanced gestational age, change in cervical dilatation & cervical effacement after two hours of admission and finally change in fetal head station after two hours of admission remained independently significant in a multiple logistic regression model for cesarean delivery. Conclusions: The use of demographic and clinical criteria early during labour in nulliparous women with vertex presentation may reduce the potential morbidity associated with prolonged labour or second stage cesarean delivery.

Outcome of labour in nullipara at term with unengaged vertex.
BACKGROUND Primigravidas with unengaged foetal head at term should be regarded as high risk cases. It is seen that nuglliparous women with the floating foetal head demonstrate higher rates of caesarean section than those with dipping or engaged head in early labour. These cases should be regarded as high risk and identified early and should be referred to a tertiary care centre where good facil...

Mid-pregnancy cervical length in nulliparous women and its association with post-term delivery and intrapartum caesarean delivery Abstract Objective: To evaluate the association between mid-pregnancy cervical length and post-term delivery and caesarean delivery during labour. Study Design: In a multicentre cohort study, cervical length was measured in low-risk singleton pregnancies between 16 a...
متن کاملWomen’s perception of pre-hospital labour duration and obstetrical outcomes; a prospective cohort study
BACKGROUND Progress during early labour may impact subsequent labour trajectories. Women admitted to hospital in latent phase (<3 cm cervical dilation) labour have been shown to be at higher risk of obstetrical interventions. METHODS We conducted a secondary analysis of data from a randomized controlled trial of 1247 healthy nulliparous women in spontaneous labour at term with a singleton fet...
متن کاملObstetric risk indicators for labour dystocia in nulliparous women: A multi-centre cohort study
BACKGROUND In nulliparous women dystocia is the most common obstetric problem and its etiology is largely unknown. The frequency of augmentation and cesarean delivery related to dystocia is high although it is not clear if a slow progress justifies the interventions. Studies of risk factors for dystocia often do not provide diagnostic criteria for the diagnosis. The aim of the present study was...
متن کاملComparison of Nulliparas Undergoing Cesarean Section in First and Second Stages of Labour: A Prospective Study in a Tertiary Teaching Hospital
Objective. We performed a prospective observational audit study to compare neonatal and maternal outcomes of the primary cesarean sections performed in first stage versus second stage of labour. Methods. One thousand three hundred and eighty-nine nullipara women who had undergone cesarean section in a tertiary teaching hospital between February 1, 2009 and January 31, 2010 were included in the ...
